These are coaching credentials, not clinical licenses.

ASSESSMENT & DATA COLLECTION

By submitting this assessment, you consent to the collection and use of all personal information you have provided, including your name, contact details, age, relationship status, and your responses to questions about personal history, trauma, substance use, abuse, self-harm, mental health, personality, behavioral patterns, and relationship dynamics.

This data is used solely to evaluate coaching fit and inform the coaching process. Your results are stored securely and accessible only to Dr. Hines and authorized members of his practice.

CONFIDENTIALITY

All information you share is kept confidential, except where disclosure is required by law, if you express intent to harm yourself or others, if there is suspected abuse of minors or vulnerable adults, in response to a valid court order, or for anonymous professional consultation with no identifying information shared.

YOUR RESPONSIBILITY

You are solely responsible for your own decisions, actions, and results. Coaching outcomes depend entirely on your effort, commitment, and follow-through. Dr. Hines provides guidance and support but makes no guarantees of any specific outcomes. You confirm you are mentally and emotionally capable of participating in this process.
